Simple yay is good for normal updates.
In some special cases, sudo pacman -Syyu or yay -Syyu may be needed, e.g. if your mirrors are changed.

Yes, it will get your system up to date. It won’t install any new packages, though. If you want those, you’ll have to install them manually.
There is nothing new on the ISO you would need to install for your system to work properly. It’s sufficient to just update it. You only need the ISO when installing EndeavourOS (or when trying to recover a broken system).
